PLEASE CREATE YOUR CLIENT PORTAL ACCOUNT BY CLICKING THE BUTTON LABELED CLIENT PORTAL
Click on “New Sign Up”
Put in your email address (the same one you gave us) and a link will be emailed to you immediately.
Check your email and click on the link to create a password you will remember (we do not have access to your password)
Log in to your Optimantra account and fill out ALL forms on your dashboard completely- must be done at least 24 hours before your first appointment
Upload any labs or imaging to your “Documents” section in the portal.
You are now ready for your appointment!
